Chronic Disease Prevention and Management in Primary Care

According to the Centers for Disease Control and Prevention (CDC), chronic diseases and conditions, such as heart disease, stroke, cancer, type 2 diabetes, obesity, and arthritis, are among the most common, costly, and preventable of all health problems in the nation. This conference will address these conditions with chronic disease experts. NPs, MDs, PAs and RNs will have a great opportunity to enhance their knowledge on prevention, guidelines, best practices and recommendations to manage chronic diseases.
